Eligibility

Is sildenafil right for you?

You may be a candidate if:

  • You are a man 18+ experiencing erectile difficulty and want clinician-guided treatment
  • You are interested in on-demand (PRN) treatment taken before sexual activity rather than a daily pill
  • You want discreet, convenient telehealth access without in-person pharmacy visits
  • You are open to clinician-guided dose optimization between standard strengths and compounded formulations

Not right for you if:

  • You currently take any nitrate medication (nitroglycerin, isosorbide mononitrate or dinitrate) — an absolute contraindication
  • You have severe cardiovascular disease, unstable angina, or a recent heart attack or stroke
  • You have severe hepatic (liver) impairment
  • You have retinitis pigmentosa or a history of non-arteritic anterior ischemic optic neuropathy (NAION)

What is sildenafil?

The science behind sildenafil.

Sildenafil citrate was discovered almost by accident. In the early 1990s, Pfizer researchers were investigating a compound originally intended to treat angina and hypertension by relaxing blood vessels in the heart. The cardiac benefits proved modest, but male participants in clinical trials consistently reported an unexpected side effect — improved erections. Pfizer pivoted the program and, in 1998, the US Food and Drug Administration approved sildenafil under the brand name Viagra, the first oral medication for erectile dysfunction. It remains one of the most widely prescribed drugs in history.

Mechanistically, sildenafil is a selective inhibitor of phosphodiesterase type 5 (PDE5), the enzyme that breaks down cyclic guanosine monophosphate (cGMP) in the corpus cavernosum of the penis. During sexual stimulation, nitric oxide is released locally and triggers cGMP production. By blocking PDE5, sildenafil allows cGMP to accumulate, which relaxes the smooth muscle of the arteries feeding the erectile tissue. Blood flow increases, the spongy tissue fills, and an erection results. Critically, sildenafil does not create an erection on its own — sexual stimulation is still required to trigger the nitric oxide cascade.

Nearly three decades of use have generated extensive clinical and real-world safety data. For most men, a standard 25, 50, or 100 mg tablet works well — but not everyone responds identically. TruVera's licensed compounding partners can prepare sildenafil in custom strengths between the standard doses or as sublingual troches (lozenges) that dissolve under the tongue for faster onset, typically 15–20 minutes versus 30–60 minutes for a swallowed tablet. That flexibility matters when a standard pill is too strong, too weak, or too slow for your life.

"I'd been putting off dealing with this for years. The intake was straightforward and my clinician took the time to explain dosing. I feel like myself again — confidence back without the awkward pharmacy run."

"The standard 50 mg was more than I needed. My clinician switched me to a compounded lower strength and it worked perfectly — fewer side effects and exactly the effect I wanted."

"Ordering through TruVera felt genuinely private. Discreet packaging, no insurance hoops, and the troches kick in faster than the tablets I'd tried before. Exactly what I was hoping for."

FAQ

Sildenafil therapy questions

What is sildenafil — is it the same as Viagra?

Sildenafil is the generic name for the active ingredient in Viagra. Pfizer developed and brand-marketed it starting in 1998, and generic sildenafil has been widely available in the US since 2017. It is FDA-approved and works by the same mechanism regardless of whether it is sold under the Viagra brand, as generic sildenafil, or as a compounded formulation from a licensed pharmacy. TruVera prescribes sildenafil in both standard generic tablets and custom-compounded forms when that's the better clinical fit.

How fast does it work?

A standard sildenafil tablet typically begins working in 30–60 minutes and lasts about 4–6 hours. A compounded sublingual troche, which dissolves under the tongue and bypasses first-pass digestion, often works in 15–20 minutes. A heavy or high-fat meal can slow absorption of oral tablets, so many patients take sildenafil on an empty stomach for the fastest and most predictable onset. Sexual stimulation is still required for the medication to produce an erection.

How is sildenafil different from Cialis (tadalafil)?

Both are PDE5 inhibitors that treat erectile dysfunction, but they have different durations. Sildenafil is shorter-acting (about 4–6 hours) and is usually taken on demand, 30–60 minutes before sexual activity. Tadalafil (Cialis) is much longer-acting (up to 36 hours) and is available both as an on-demand dose and as a low daily dose for continuous coverage. The right choice depends on how often and how spontaneously you want the medication to work — your clinician will help you decide.

What are the side effects?

The most common side effects are mild and usually transient: headache, facial flushing, nasal congestion, indigestion, and temporary mild vision changes (a blue-tinged hue or light sensitivity). Rare but serious effects include priapism (an erection lasting more than 4 hours), which is a medical emergency requiring immediate care, and sudden hearing or vision loss. Stop taking sildenafil and seek care immediately if any of these occur.

What drug interactions do I need to know about?

The most critical interaction is with nitrates — including nitroglycerin, isosorbide mononitrate, isosorbide dinitrate, and recreational "poppers" (amyl or butyl nitrite). Combining sildenafil with any nitrate can cause life-threatening hypotension and is an absolute contraindication. Alpha-blockers (often prescribed for blood pressure or prostate enlargement) require caution and dose separation because they can lower blood pressure when combined with sildenafil. Certain antifungals, HIV protease inhibitors, and some blood pressure medications can also affect how sildenafil is metabolized. Always disclose every medication and supplement you take during your intake.

Can I pause or cancel my subscription?

Yes. You can pause or cancel at any time through your TruVera patient portal — no phone calls, no cancellation fees. If you want to try a different dose, switch between tablets and troches, or explore another ED therapy before canceling, your care team is available to discuss options.
